Group of fields:

Can be used to group together anyquestions related to one topic. Here you can see an example of two Text boxes inside one Group 

Text box:
An area where you can put even a big text.
You can spesify who exactly must  the question be Answered by: employee, manager or both.
Put Final decision checkbox if it is necessary to come to a consensus after discussion or someone might change their mind during the talk.

Input numbers only. Use Preffered value to build a nice report after the talk.

Use checkbox for multiple choise questions. Add or remove options to get exactly what you need. All non-text fields in template can be commented during the talk if you wish.

GIves user an opportunity to chose only ONly one answer of the options given.

Radio group
Collection of several radios. If you put N/A(not applicable) user will get a chanse not to answer this question and this won't be counted in statistics.

Add a drop of brightness to your day by using smilies in your talk. You can have three to five options :-)

Drop down
Pretty much the same as the checkbox but a different view.
One or more options can be selected.

Usually used for ratings. You can set your own Min and Max value.
And get a nice report using preffured one.

Multi slider
A set of slider where you can add more than ne question.

Upload any picture to make your template nicer or include visual description/example to help user understand the question.

Element desription
Input any text for user to read: instructions, descriptions, examples, inspiration!

Data import
Attach  personal user data to the talk so that it is easy to see and discuss KPIs, Salary, etc(only for companies who have DSM module enabled)


The elements existing in evaluation are same as in talks. The differense is that it usally requires sevsral people so everyone answer every question and no final decision can be made.


In surveys users can only comment their answers if this setting is checked.
